 The list of (new old) members of the Institute / Unit   sent by the Council to the Life Insurance Corporation   of India for the year 2022-2023

 परिषद द्वारा संस्थान/ एकक के (नये पुराने) सदस्य की सूची जो   भारतीय जीवन बीमा निगम को भेजी गई सूची वर्ष 2022-2023   के लिए

 i. List of Officers/employees of the Council who are  members/Covered Central Government Employees  Group Insurance Scheme (CGEIS) through of Life Insurance Corporation of India (LIC)

(List shorted Unit/Institute State-wise Alphabetically)

परिषद के अधिकारी/कर्मचारी की सूची जो भारतीय जीवन बीमा निगम (LIC) के सदस्य

 केवल नियामित कर्मचारी ही समूह बीमा योजना के सदस्य हेतु

 (Only for Regular Council’s Employee)

 GIS membership is mandatory

 ii सदस्य द्वारा प्रार्थना पत्र। जिसमे

An application by Employee.

 iii. नियमित होने का कार्यालय आदेश।

Office order of regularization service.

 iv. नाम एवं जन्म तिथि को सत्यापित करने के लिए Matric एवं equalization certificate।

 v. Date of Birth Proof (Matric or equalization certificate)

 कर्मचारी परिवार के लिए नामांकन नामांकित/ नामांकित परिवार के एसडीएस का परिवर्तन (Nominee’s/ Change of  Nominee/ Beneficiary/Beneficiaries)

 i. नामांकित (Nominee) फॉर्म (Appendix-II)

 ii. Form of Agreement

 Form for Death Claim from LIC As per Council’s GIS   record मृतक के नाम ओर मृतक के Death Certificate मे   नाम भिन्न होने पर नीचे दिया हुआ Affidavit देना अनिवार्य है

 i. नियोक्ता/दावेदार का विवरण

(Employer’s/Claimant’s Statements)

 ii. Discharge Receipt

 iii. मृत्यु प्रमाण पत्र (मूल) Death Certificate (Original)

 iv. लाभार्थी/लाभार्थियों के आधार कार्ड की फोटोकॉपी (Photocopy of Aadhar Card Nominee’s/ Beneficiary /Beneficiaries

 v. लाभार्थी/लाभार्थियों बैंक खाते का विवरण

(Bank account details Nominee’s/ Beneficiary /Beneficiaries)

 vi. यदि मृत्यु प्रमाण पत्र में उल्लिखित नाम जी॰ आई॰ एस॰/ एल॰   आई॰ सी॰ के कार्यालय मे

 भेजी गई सूची से मेल नहीं खाता है तो एक

 शपथ पत्र जमा भी साथ में जमा करें।

 (If death certificate name mismatch with GIS list sent to  LIC office by the Council, so submit an affidavit for name correction mandatory)
